The Rare Disease Program through MHealth Fairview Pediatrics provides care to individuals and families with a possible or confirmed rare disease diagnosis.

The Rare Disease Program through MHealth Fairview Pediatrics works together with other pediatric specialty clinics to provide evaluation, care, and support to individuals and caregivers with a possible or confirmed rare disease diagnosis. This care includes support from a patient navigator or care coordinator, who helps guide the individual and family through the process of evaluation and treatment. The care team may include a genetic counselor, social worker, child-family life specialist, registered dietician, financial case manager, and specialty care doctors.
